Your First Visit

Upon your visit to Premier Heart and Vein Care, you can expect first-class care from our staff right away.  We’re with you every step of the way to ensure your experience is nothing less than easy!

Patient Forms

On your first visit our  staff will  provide a warm welcome to make your first visit as comfortable and stress free as possible.  When you arrive, you will need to complete our  patient forms. Venous Ultrasound Exam

Next, vein patients will have an ultrasound exam in our state-of-the art, certified lab which will provide a road map to determine your best plan of treatment.


Consultation & Treatment Plan

You will have a consultation and physical examination with Dr. Stevens to discuss your problems and concerns. This initial visit is designed for him to learn more about you, your condition, and expectations. Previous medical records will be requested as well. Dr. Stevens will then provide you a treatment plan to optimize your care. Sometimes patients will have testing done prior to seeing Dr. Stevens so your care is provided as expeditiously as possible
